This paper provides a forensic investigation into the systemic causes of project management failure, focusing on administrative opacity and the lack of independent oversight. By introducing the Construction Management Integrity Metric ($CMIM$), we quantify the operational cost of governance breakdowns. Our forensic analysis of 95 commercial projects indicates that 84% of management failures are linked to the absence of independent technical auditing during the design-to-execution transition. We propose a Forensic-Integrated Project Governance ($FIPG$) framework, arguing that objective, data-driven third-party oversight is essential for maintaining project ROI.
