This paper conducts a comprehensive forensic analysis of mega-project disputes, identifying the recurring failures in contractual governance and technical risk allocation. By integrating longitudinal project data with failure mode analysis, we introduce the Mega-Project Dispute Index ($MPDI$). Our research, based on 50 mega-projects, reveals that 89% of disputes stem from "pre-construction myopia," where expedited procurement outweighs essential geotechnical and topographic verification. We propose a Forensic-Integrated Governance ($FIG$) framework to standardize conflict mitigation, emphasizing independent third-party auditing as the primary mechanism for protecting asset value.
